Adenoidectomy is a surgical procedure to remove lymphatic tissue behind the nose called
adenoids.
Like we have two tonsils in the mouth, there is another group of lymphatic tissue behind the nose
and protecting the airway called adenoids. These are a part of our immune system and help to stop the
infection from flaring up in our body. Generally enlarge around 2 to 3 years of age and grow upto puberty
often leading to obstruction of breathing passages, later the growth of skull overtakes the growth of these
tissues thus normalizing the airway space.
They get inflamed or enlarged during acute bouts of infections in the nose or throat, often leading
to mouth breathing, snoring, blocked ears leading to reduced performance at school. If the persistant
enlarged adenoids remains untreated, leads to elongated upper half of the face, high arched palate,
crowding of teeth, these features typically called adenoid face.
Surgical removal of adenoids is an option to consider after giving a trial of medical treatment for
about 8 weeks which include a local corticosteroid nasal spray and an anti-allergic.
Mouth breathing, snoring, blocked ears leading to reduced performance at school. On examinations, there is elongated face appearance, High arched palate and Crowding of teeth.
Clinical examination is followed by a Lateral Neck X ray and sometimes nasal endoscopy.
Many times wait and watch maybe the best option and most of the times the adenoid issue gets better with age.
But in persistent cases, a trial of medical management with corticosteroid nasal sprays and anti allergic medications can be tried before proceeding for surgery.
Coblation Adenoidectomy is a scarless surgery performed with the use of an endoscope introduced through the nose along with powered instruments (Coblator) used to shave off the adenoid tissue, the advantage of this when compared to the traditional blind technique of curettage being less trauma to the surrounding normal tissues, complete and painfree removal of adenoids and lesser complications post surgery.
The surgery is done under general anesthesia and it takes about 10 to 15 minutes to be completed after the patient is under anesthesia. It can be done as a daycare procedure where the patient walks home about 4 hours after recovery from anesthesia and can resume regular activties after 2 days, provided he/ she wears a mask and avoids dust and smoke.
